The KSLC Navigation Team

A program of Hyosarang Mission. Hyosarang Mission is the accountable nonprofit home; the Korean Senior Life Campus (KSLC) Navigation Team is its specialized function, operating under Board-approved oversight, written procedures, and measurable outcomes.

One accountable home, one specialized team

  • Hyosarang Mission holds legal, fiscal, and programmatic responsibility.
  • KSLC runs screening, application support, warm handoffs, and follow-up.
  • Trusted outreach leads to services seniors actually receive.

The five-step navigation process

  1. 1

    Intake & Screening

A brief Korean-language conversation identifies priority needs across benefits, housing, food, transportation, and digital access. Consent is obtained and a secure Case ID is assigned.

  2. 2

    Action Plan

Clear next steps based on urgency, eligibility, and readiness — with a named responsible person and a due date the participant agrees to.

  3. 3

    Application Assistance

We organize documents and support benefit, housing, and service applications, renewals, and responses to notices.

  4. 4

    Warm Handoff

With consent, we connect the senior directly to a named agency or qualified partner and record the contact, referral date, and next step.

  5. 5

    3 / 14 / 30-Day Follow-up

We check back at 3, 14, and 30 days to confirm the senior actually reached the service, record any barrier, and escalate when needed.

Five core barrier areas

Public Benefits

Medi-Cal and Medicare information and qualified referral pathways; IHSS, SSI, CalFresh, and utility support; applications, renewals, and notices.

Housing

Affordable-housing information, opportunity tracking, application readiness, and housing-navigation pathways.

Community members sharing a meal together

Food Security

Nutrition resources, food programs, and trusted connections, checked against existing services.

Transportation

OC ACCESS, city senior-mobility programs, OCTA, and community rides — with non-duplication checks — for education, benefits, and essential services.

Seniors practicing on smartphones in a digital-literacy class

Digital Access

Email, online portals, document uploads, online forms, and safe online participation.

Privacy by design

Data protection is built into the workflow, not added afterward.

  • A secure Case ID for every participant
  • We collect only what a service needs
  • We explain how information will be used
  • Access is limited by role
  • Reporting is de-identified and aggregate only
